Q. What bills can I expect to receive after surgery?

A. If surgery is required, separate statements will be sent to you for each provider of services. You may receive billing from each of the following:

  • Physician
  • Hospital or surgery center
  • Surgical assistant
  • Anesthesiologist
  • Pathology/lab
  • Radiologist
  • Durable medical equipment (DME) providers
Q. Does Missoula Surgical Associates contract with my insurance company?

A. We are participating providers with many insurance companies. However, to ensure that we are contracted with your specific health plan, please contact your insurance company prior to any appointments to verify eligibility and coverage.

Q. Does my insurance company pay for any procedure performed at any facility?

A. You will need to contact your insurance company for any facility restrictions.

Q. I just received my bill and I cannot pay the full amount. What should I do?

A. Please call our Patient Accounts Department at 406.829.2388. They will be happy to discuss payment options with you.

Q. Can I pay my bill with a credit card?

A. Yes. We accept VISA, MasterCard, Discover and American Express and can take payment in our office or via our website under Pay Your Bill

Q: Do you accept Medicare Assignment?

A. Yes.

Q. How do I obtain a refill of my prescription?

A. Please contact your pharmacy and have them fax a refill request to our office at 406.829.0661. You may also request a refill via MyChart.

Q. Can a friend or family member come to my appointment with me?

A. We encourage you to bring a family member or friend to your appointment, as you see fit. It can be beneficial to have an extra set of eyes and ears and additional support.

Q. How do I obtain a copy of my records?

A. You can download a printable version of Missoula Surgical Associate’s Release of Information Form from this website. Complete this form and either mail, fax (406.829.0661), or bring it to our office.

Q. What should I bring to my first appointment?

A. You will need to bring the following to your first appointment:

  • Patient paperwork completely filled out (if you did not receive paperwork in the mail, please arrive 30 minutes early)
  • List of your current prescription medicines
  • Photo ID
  • Insurance card(s) or if you do not have insurance, a $150 down payment for the consultation fee
Q. Where are you located?

A. We are located at the 2819 Mullan Medical Center Building. Turn off Mullan Road onto Great Northern Loop by the entrance to Missoula Bone & Joint. Head west and we are the 3rd building on the left, located on the 2nd floor.

Q. Do the surgeons of Missoula Surgical Associates perform any cosmetic surgery? Such as breast reduction, enhancement, etc.?

A. The surgeons at MSA do not perform any cosmetic surgeries. We encourage you to speak with your primary care provider to see who they recommend for this type of surgery.

Q. I have Family Medical Leave Act (FMLA) paperwork, who should fill this out?

A. Your doctor and nurse are available to fill out your FMLA paperwork. Patients are urged not to fill this form out by themselves, as it will be deemed invalid if not filled out by the doctor’s office.
